The CA125 test measures the amount of the protein Cancer Antigen 125 in your blood. It is primarily used as a “tumor marker” to help monitor how well treatment is working for ovarian cancer or to check if the cancer has returned after treatment. Does a high CA125 level mean I have cancer?
No, not necessarily. An elevated CA125 level can be caused by many non-cancerous (benign) conditions. Only about 50% of women with early-stage ovarian cancer have high levels, and many women with high levels do not have cancer at all. This is why the test is usually interpreted alongside ultrasounds or physical exams.
What are the common non-cancerous causes of a high CA125?
Several common conditions can cause fluctuations in your CA125 levels, including:
Endometriosis or uterine fibroids.
Pelvic Inflammatory Disease (PID).
Pregnancy or your regular menstrual period.

When is the best time during my cycle to take the test?

Do I need to fast for this test?

What is considered a "normal" range for CA125?
Generally, a level below 35 U/mL is considered within the normal range. However, “normal” can vary based on age and menopausal status. Postmenopausal women typically have much lower baseline levels than premenopausal women.
Why isn’t CA125 used as a general screening tool for everyone?
The test is not used for general screening because it can produce “false positives” (showing high levels when no cancer is present) and “false negatives” (showing normal levels when early cancer is present). It is most effective for women who already have symptoms or a strong family history.
